Position Summary:

The OB Care Navigator will be a key member of the perinatal care team, working closely with obstetric providers, nurses, behavioral health clinicians, and population health managers. This individual will provide patient-centered, comprehensive care coordination and resource navigation for pregnant and postpartum patients, with a special focus on those facing complex medical, social, and behavioral health barriers.

  • Actively engage with pregnant and postpartum patients to assess needs related to social determinants of health (SDOH), mental health, and clinical care gaps.
  • Facilitate warm hand-offs to behavioral health, nutrition, lactation support, WIC, and community-based services.
  • Create and document individualized care plans in collaboration with the patient, OB provider, and multidisciplinary care team.
  • Provide education on accessing and navigating health services, insurance support, and community resources (e.g., food security, housing, utilities assistance, childcare, transportation).
  • Participate in weekly multidisciplinary high-risk rounds and contribute to case discussions and follow-up plans.
  • Collaborate with legal service agencies and educate patients on their rights as tenants and workers.
  • Track referral completion and conduct follow-up outreach to ensure patients receive needed services
  • Maintain updated knowledge of community resources and referral pathways.
  • Document patient interactions and navigation outcomes in the electronic medical record (EPIC).
  • Participate in ongoing training in trauma-informed care, implicit bias, and culturally and linguistically appropriate care.
  • Coordinate care for high-risk OB patients, including scheduling assistance, specialty referral support, and follow-up tracking.
  • Participate in monthly meetings with OB department leadership to discuss care navigation workflows: What is going well? What could use improvement?
  • Maintain excellent punctuality and attendance.
  • Promote a sense of “teamwork” through demonstration of self-direction and self-motivation.  Solve problems independently or knows when to seek consultation.
  • Displays outstanding customer service skills when interacting with all NeighborHealth customers according to the ACE model.
  • Adheres to all NeighborHealth and departmental policies and procedures.
  • Participate in required departmental activities and meetings.
  • Perform other related duties.

EDUCATION:  Bachelor Degree preferred.

EXPERIENCE: Minimum of 1-2 years in care navigation, community health, case management, or OB care coordination. Experience with and underserved population a plus.

Skills/Abilities: 

Strong second language skills.  Spanish preferred.

Strong communication, organization, and problem-solving skills

Strong knowledge of Microsoft Office

Hourly rate $22.00 up to $33.00 based on experience.
