Temporary - Support Coordination Spec (Administrative)

Temporary Role - August - September - Supports Coordinator

Provides an array of support services to intellectually and developmentally disabled adults. Utilizes person centered planning values/processes to help individuals assess their needs and develop individual plans of service. Monitors individuals progress toward achieving specified goals and objectives. The process involves identifying support strategies to include coordinating and linking individuals to support services in their community and advocating to ensure service needs are met. Process also includes empowerment, community inclusion/integration, health and safety assurances, and the use of natural supports. It involves coordinating the individuals care with other behavioral and mental health professionals. Performing these duties to the standards established by the organization as well as its funding sources and regulatory bodies.
